Cyclosporine-associated neurotoxicity. The need for a better guide for immunosuppressive therapy.

چکیده
Heart transplantation has become the most effective treatment of end-stage heart failure in selected patients. More than 30 000 heart transplantations have been performed worldwide since its introduction in 1967, with a current 1-year survival rate of 85% and 5-year survival rate of 70%.1 Besides the enormous survival advantage associated with heart transplantation compared with all other therapies, it also usually is associated with the restoration of a nearly normal age-predicted functional capacity.

بررسی پلی مورفیسم ژن های CYP3A5، MDR1 و ارتباط آن با غلظت خونی سیکلوسپورین در مراحل اولیه بعد از پیوند کلیه در بیماران شیراز
Background & Aims: The immunosuppressive drug cyclosporine with a narrow therapeutic range and variable pharmacokinetic characteristics among individuals is a substrate for cytochrome P450 (CYP) 3A and P-glycoprotein, the product of the multidrug resistance 1 (MDR1) gene. Some of the single nucleotide polymorphisms (SNPs) in these genes are associated with deficient protein expression and red...

متن کاملGingival Overgrowth in Subjects under Immunosuppressive Regimens Based on Tacrolimus or Combination of Tacrolimus and Amlodipine
Tacrolimus has been used successfully to prevent acute rejection of renal transplantation. It inhibits calcineurin, a Ca2+ calmodulindependent phosphatase, which results in reduced expression of interleukin-2 by T lymphocytes [17]. Tacrolimus has been associated with many side effects including nephrotoxicity, neurotoxicity and the induction of a diabetic state. Unlike cyclosporine A, studies t...
متن کاملEvaluation of individual and combined neurotoxicity of the immunosuppressants cyclosporine and sirolimus by in vitro multinuclear NMR spectroscopy.
Neurotoxicity, a crucial side effect of immunosuppressive therapy with cyclosporine, also has been demonstrated in vitro for sirolimus, a novel macrolide immunosuppressant, which is under clinical investigation in combination with cyclosporine. NMR spectroscopy was used to study the separate and combined effects of cyclosporine and sirolimus on cerebral metabolism, both in brain cells and in pe...
متن کاملNeurotoxicity of immunosuppressive therapies in organ transplantation.
ABSTRACT Immunosuppressive agents have revolutionized clinical transplantation medicine, allowing the avoidance of immune system attack on the transplanted graft. Nevertheless, the use of medications such as cyclosporine, tacrolimus and others also brought the side effects of these drugs. Early identification of drug-induced neurotoxicity in transplanted patients and of its specific causes is i...
